Enrollment Center Registration Process

The following items are required to enroll your student.  Missing information will delay the placement of your child.

  • Legal evidence of age: birth certificate, baptismal certificate, passport, immigration certificate, Bible record or affidavit from the parent/guardian.
  • Proof of residency: current utility bill in parent's name showing service address (gas, electric, water and/or trash only - no "Disconnect" notices); rental agreement and current rent receipt; closing escrow papers; or an approved Inter-district transfer. Cable and phone bills, driver's license and other DMV documents are not acceptable.
  • Current immunization record.
  • Report of Health Exam for School Entry for Kindergarten and 1st grade.
  • For grades 6th through 12th, transcripts/most recent report cards and if enrolling during the school year, a drop slip with last date enrolled.
  • Special Education/Special Needs documents - IEP (RSP, SDC, Speech, and SH/SDC), GATE, Bilingual. Registration of Special Education students without proper documentation may delay placement.
  • Parent/Guardian photo I.D.
  • Legal custody or court orders, if applicable
  • If you and your child are living full-time with another family, please bring that person with you along with their proof of residence and photo I.D. to complete a Declaration of Residence form.
